Mental Health Assist in Newton, Sussex County, New Jersey

Overview
Newton Medical Center, a member of the prestigious Atlantic Health System is currently seeking a FT mental health asst. in the O/P Behavioral Health unit.

Responsibilities
Working in collaboration with Clinician and Child Psychiatrist to provide care management resources to pediatricians and families.
Resource identification for youth with behavioral health needs.
Phone communication with Pediatricians and their office staff.
Participate in weekly Hub team meetings.
Communicate with the other Expanded Service Hub team via telephone or web conference.
Work with PCP practices within their respective Expanded Service Hub area. Participate in any MHC/St. Joseph sponsored webinars and/or trainings.
Input required information into Expanded Service HUB database and tracking system.
Phone assessment of youth who screen positively for behavioral health concerns in pediatrician's offices.
Care coordination with pediatrician and families.
Promotion of Morris/Sussex HUB services

Qualifications
EDUCATION:

• Bachelors degree in Psych., counseling or human services field.
• 1-year experience working with children/adolescents in a behavioral health related area. Excellent communication skills.
  Computer literate with proficiency in Microsoft Office.
  Ability to work in an interdisciplinary team setting.
  Strong organizational and time management skills
