Optima Health Community Care is hiring a Registered Nurse (RN) Team Coordinator to lead our team of dynamic care coordinators and conduct face to face assessments with our valued members and coordinate their care as a part of our DMAS contract. This region consists of Harrisonburg, Charlottesville, and Waynesboro regions.
$5000 sign on bonus for qualified candidates

Department/Position Overview:
This essential position requires:
  • Registered Nurse (RN Certification) and a minimum Bachelors Level Degree in Nursing.
  • Working in a home-based office capacity (equipment provided).
  • Supervising 15-20 care coordinators.
  • Averaging about 3-4 days per week out in the field.
  • Travel distance for cases and visits within an average 60-80 mile radius.
  • Mandatory training for one week in Hampton Roads area of Virginia.
  • Minimum 1 year nursing experience working with long term and support services populations.
  • Field based care coordination/case management and/or home health experience preferred.

Assumes responsibility, accountability and leadership for the daily operations including coordination of work, quality, and service. First line supervisor in the Department of Medical Care Management for assigned site/function. Facilitates the work of assigned team members. Provides a leadership role in ongoing case manager competency assessment, needs identification and educational offerings. Provides educational services to the Medical Care Management staff . Participates in the work activities of assigned teams and provides case management services as needed.

Education Level
RN-Bachelor's Level Degree

Experience
Required: Case Management - 3 years

Preferred: Supervisory - Previous experience

License
Required: Registered Nurse

Preferred: Basic Life Support

Skills
Required: Communication, Critical Thinking, Service Orientation

Other
For Integrated Care Management departments, specialty certification required within one year of eligibility (ACM, CCM, CCCTM or RN-BC). For other service lines, certification based on specialty area required within one year of eligibility. BLS (if in a clinical setting).
